The Harold & Grace Baker Centre Retirement Residence is licensed by the Retirement Homes Regulatory Authority (RHRA), an arm’s length regulatory authority created by the Province of Ontario under the Retirement Homes Act, 2010 (Act). The RHRA is responsible for licensing and inspecting retirement homes as well as overseeing compliance and enforcing the Act for the protection of residents.

The Retirement Residence also meets the comprehensive set of operating standards set by the Ontario Retirement Communities Association (ORCA). Established in 1977, ORCA is a non-profit, voluntary professional association that represents more than 420 retirement residences province-wide. ORCA sets professional operating standards, inspects and accredits Ontario-based retirement residences.

At the Harold & Grace Baker Centre, our residents are at the heart of everything we do. We are committed to helping seniors live life to the fullest by providing them with choices to meet their unique personal care preferences.

Our goal is to deliver outstanding personalized care and service. Registered staff work with every Baker Centre resident to conduct a LiveWell™ Care and Service assessment which is used to develop a personal plan that is reviewed on a regular basis.

LiveWell™ is our easy to understand and manage care program that allows residents and their families to customize care needs by choosing only those care options desired and needed.

The LiveWell™ care plan respects and responds to individual needs. Our approach is to work collaboratively with residents and families to achieve success.

Person-centred care promotes safety, quality and continuous improvement of services. We offer a full range of care options and focus on finding solutions that our residents need and desire.

To help our residents keep living well, the Baker Centre offers the LiveWell™ Personalized Care Program as well as a number of signature Revera programs:

Personalized Care Program

LiveWell™ provides care themes around areas such as Nutrition, Medication, Mobility, Bathing, Continence and Personal Assistance. Each theme has varying levels of assistance that allows for a personalized approach to care. You choose only those areas where assistance is required or desired.

The Harold & Grace Baker Centre is pleased to use Revera’s LiveWell™ Personalized Care Program, a well-researched and unique model for managing care in the retirement sector which allows for multiple provisions in resident care.

LiveWell™ is about personalized service and meeting the individual needs and preferences of our residents. By choosing only those care services required, we are able to build a more personalized program tailored to individual needs. A comprehensive LiveWell™ assessment is completed by our Director of Health and Wellness to ensure consideration of each resident’s needs. Residents will then have the flexibility to choose from a list of care themes or services to create their own personalized care plan.

We fully understand that, as our residents age, some may find themselves in need of additional care in order to maintain the full quality of life to which they have been accustomed. LiveWell™ is designed to be flexible – as a resident’s needs change, their LiveWell™ program will change with them. Care plans are reviewed on a regular basis by our Director of Health and Wellness and care staff.

Montessori Programs

A progressive, personalized approach that enables residents with memory loss to function at their best.

Falls Intervention and Risk Management

FIRM offers practical and effective strategies and methods to reduce the risk of falls.

When it comes to reducing falls, the Harold & Grace Baker Centre takes a FIRM approach. FIRM stands for Falls Intervention and Risk Management.

This is done by assessing and addressing risks (from footwear to medication); offering education (to residents, families and staff) on preventing falls; providing strength and balance therapy; making necessary modifications (from bed height to lighting); and adopting other best practices.

Award-winning Revera/3M Skin Health Program

That supports optimal skin care to prevent skin wounds.

Skin and wound care is a vital part of overall health, and preventing the development of issues such as pressure ulcers is critical to healthy aging. Through a partnership with 3M Health Care, Revera developed a best practices program that has achieved excellent results.

The partnership with 3M started in 1999 and offers consistency in managing skin and wound issues; the highest quality products (supported clinically through evaluation and research); a standardized method of tracking results; and ongoing education for health and wellness staff to have the latest training in this area.

The award-winning Revera/3M program continues to be seen as an industry leader in delivering the best skin and wound care protocols, practices and outcomes for long term care residents.

Dining

One of the greatest pleasures of living at the Harold & Grace Baker Centre is the dining experience

The culinary staff combines creativity, high quality ingredients and first class service to create delicious, healthy meals.

All meals are prepared fresh daily by an on-site cook, ensuring that the menu is balanced, flavourful and nutritious. Two choices are available at every meal.

Along with the daily variety, there are themed menus for holidays, and favourite seasonal fare. Residents also enjoy tea, coffee and refreshments twice daily on the second-floor Steele Lounge.

Facilities

The 92 suites of the Retirement Residence are housed in the upper four floors of the central 6-story building

The lower two floors include the spacious entrance lobby with plenty of sitting areas, the brightly-lit main dining room, the Memory Lane gift shop which stocks toiletries and a selection of giftware, a library, a chapel, a hair salon, and the Steele Lounge which is used for entertainment, movie nights, cocktail hour and tea and coffee service.

The suites in the Retirement Residence have large windows that overlook the beautiful grounds.

All accommodations include three meals a day, snacks, housekeeping, linen and linen laundry, weekly visits by a physician, 24-hour Registered Practical Nurse (RPN) on-site and recreational programs.
