Continuum of Care

Distinctive in that it is both a Retirement Residence and a Long-Term Care Home

Residents can be assured that, as their care needs change, the Baker Centre can seamlessly support the transition through a wide range of service and care programs.

The Baker Centre is one of the few “multi-level” homes in the Greater Toronto Area. A multi-level home offers both a Retirement Residence and a Long-Term Care Home. Unlike many retirement-only residences, the Baker Centre Retirement Residence offers higher levels of care as needs increase.

So, as their needs become more complex, residents can choose from a list of care themes and services to create their own personalized care plan.

This continuum of care provides peace of mind for residents and their families enabling them to make the choice to stay in their home at the Baker Centre.

Quality of Care

Quality is the foundation of everything we do at the Baker Centre

Quality targets and outcomes are reviewed quarterly at our Quality Council. The challenging targets for quality of care are set and monitored by an attentive and dedicated volunteer Board of Directors with considerable and varied senior executive experience in the private sector and the healthcare industry.

In 2017, the Baker Centre transitioned to CARF accreditation resulting in a Three-Year Accreditation. Through this process the home has further enhanced and refined the quality program to include: risk management, cultural diversity, accessibility, IT and strategic planning. These plans are developed and shared with residents, families, staff and external partners to support the home’s priorities, targets and activities.

The Harold & Grace Baker Centre strives to meet or exceed the standards set out by the Ontario Ministry of Long-Term Care through the Resident Quality Inspection process. The Baker Centre Long-Term Care Home meets the accreditation standards set by CARF International while the Baker Centre Retirement Residence meets the requirements of the Retirement Homes Act, as monitored through the Retirement Home Regulatory Authority.

Caring Staff

Known and respected for its friendly, caring and highly motivated staff

Expert professional management and leadership is provided by Revera Inc., an industry leader with many years of management experience in the geriatric care field.

Staff turnover is low, resulting in experienced employees with superior knowledge of and relationships with residents and responsive communication with each other as well as with residents and their families.

In addition to its caring staff, a broad base of committed volunteers provides support and assistance on a daily basis.

Affordable Rates

Not-for-profit corporation, offering quality care at affordable rates

As with all long-term care homes in Ontario, rates for the Baker Centre Long-Term Care Home are set by the Ontario Ministry of Long-Term Care. However, the rates for the Baker Centre Retirement Residence are set by the Baker Centre itself and annual rent increases are governed by the Landlord and Tenant Act. Since the Baker Centre is a not-for-profit corporation, we are able to offer affordable rates.

Governance

Not-for-profit governance model

The Harold & Grace Baker Centre is governed by an independent, volunteer Board of Directors. Board members have extensive backgrounds in such professions as engineering, architecture, construction, health care, accounting and law. As volunteers, they provide their expertise willingly and without compensation.

Revera Inc. is one of Canada’s leading providers of seniors’ accommodation, care and health services. The Board of Directors engages Revera to manage Baker Centre employees thereby leveraging the expertise and best practices of a leader in the field.

As a not-for-profit corporation, the Baker Centre offers affordable rates to residents and, after covering all operating expenses, retains a modest balance for capital projects. The Board of Directors re-invests any surplus funds and charitable donations in priority capital projects for the safety and comfort of Baker Centre residents.
